Contact information includes residential address, telephone number, and a functional electronic mail address through which administrative correspondence shall be discharged. Emergency contact information includes the name, telephone number, and electronic mail address of a designated point of contact resident outside the United States. Employment information, where applicable, includes the name, address, and telephone number of the present employer and the position or job title held.

Eligibility declarations encompass responses to enumerated questions concerning communicable disease status, criminal history, prior immigration matters, terrorism- related concerns, and other matters bearing upon admissibility.
